Passports

More people than ever will soon need a passport to travel anywhere outside the United States borders, including Bermuda, the Caribbean and Canada.  For more information,  CLICK HERE.

SPECIAL NOTICE:
Regular service is taking 6 to 8 weeks from the time the application is received by the passport agency.  Expedited service is taking up to 3 weeks. For an additional $16.25, applicants have the option of having their passport sent back to them by overnight express mail.  See #7 below  for more details.

Passport applications are accepted at any of these four locations:

  • 425 North Orange Avenue, Room 355, Orlando
  • 450 North Lakemont Avenue, Winter Park
  • 1111 North Rock Springs Road, Apopka
  • 475 West Story Road, Ocoee

**Shortest wait time at the downtown courthouse is in the morning between 7:30 am and 9:30 am. Shortest wait time at all other locations is between 8:00 am and 11:00 am. Please arrive and sign in by 4:30 pm to allow us time to process your application.

Requirements for a U.S. Passport

  1. Application Form DS-11
    Fill out DS-11 "Application for Passport". Read the form carefully. http://travel.state.gov.
      
  2. Proof of U.S. Citizenship:
    • Certified copy of Birth Certificate from state/county in which you were born, no hospital certificates with footprints. If you need to obtain a certified copy of your birth certificate you can visit: http://www.cdc.gov/nchs/howto/w2w/w2welcom.htm OR
    • Naturalization Papers, OR
    • Citizenship Certificates, OR
    • Previously issued expired passport. If your name has changed on your previous passport then you must submit the legal document showing the name change. (Marriage license, divorce decree, etc.)
    • If applying for a child age 15 or under, please see the heading below titled  If applying for a child age 15 or under: 
        
  3. Photo Requirements:
    • You must supply two photographs. 
    • The photographs must measure 2 x 2 inches.
    • They must be identical to each other.
    • They must have been taken within the past six months.
    • They must show your current appearance.
    • They must be color.
    • They must be full-face, front-view photographs with a plain white or off-white background.
    • They must show between 1 inch and 1 3/8 inches from the bottom of your chin to the top of your head.
    • They must show you wearing normal street attire:
      • Uniforms should not be worn in photographs unless the uniforms consist of religious attire which you wear daily.
      • Do not wear a hat or headgear that obscures your hair or hairline.
      • If you normally wear prescription glasses, a hearing device, a wig or similar articles, these should be worn for your photograph.
      • Dark glasses or nonprescription glasses with tinted lenses are not acceptable unless you need them for medical reasons. A medical certificate may be required.
  4. The downtown office, Winter Park, Apopka, and Ocoee Branches offer a photo service for a fee of $10.00.
     

  5. Valid Identification:
    Driver's license or Military ID issued over six months ago. State ID cards are acceptable only with several other forms of ID. Parents getting passports for their children must bring proper documents showing they are the father, mother or legal guardian. (Long form birth certificate, adoption papers, etc.)

  7. PASSPORT BOOK
    • Age 16 and older: $100.00 (valid 10 years)
    • Age 15 and under: $85.00 (valid 5 years)

    PASSPORT CARD (This card is new and will go into production in Spring 2008.  You many not expedite this card at this time.)
    • Age 16 and older: $55.00 (valid 10 years)
    • Age 15 and under: $35.00 (valid 5 years)

    PASSPORT BOOK & CARD (Apply for these at one time)
    • Age 16 and older: $120.00 (valid 10 years)
    • Age 15 and under: $95.00 (valid 5 years)

      Payment must be made as follows:

    • Clerk of Court $25 Cash, Money Order, Master Card or Visa only.
       
    • For a passport book $75.00; for a passport card $20.00; for BOTH a book and a card $95.00:  U.S. Department of State (Age 16 and older) effective 02/01/2008 - check or money order only.
       
    • For a passport book $60.00; for a passport card $10.00; for BOTH a book and card $70.00:  U.S. Department of State (Age 15 and under) effective 03/08/2005 - check or money order only.
       
    • Expediting is an additional $60.00 per person, payable to U.S. Department of State, plus an additional $16.25 for return overnight delivery (cash and credit cards not accepted).

  1. Appearance
    All applicants must appear in person. Applications for children age 15 and under require signature of both parents. (see information below for applying for children age 15 and under) Applicants ages 16 through 17 must appear in person with a parent unless the applicant has a regular driver's license issued over six months ago.

     
  2. Passport Issuance
    You should receive your passport in approximately 6 to 8 weeks from the time your application is received by the passport agency.  If you wish to receive your passport sooner, you may request expedited service.  With this service and with additional mailing costs, you should receive your passport within 3 weeks.  In order to do this, the additional costs include an expedited passport fee of $60.00, plus the price of overnight express mail one way, which is $16.25, which can be included in the check or money order to the US Dept of State.

Renewal by Mail Passports

Please note: The clerk cannot accept any of the forms indicated below. The following information is provided for information only.

Fill out DS-82 "Renewal by Mail" (Purple Form). Read the form carefully. You must qualify to use DS-82. http://travel.state.gov

Submit your last passport. Your last passport must have been issued within the last 15 years, and you must have been 16 or older when your last passport was issued.

Two identical 2" x 2" passport photos. (See the yellow pages.)

Renewal fee of $75.00 payable by check or money order to U.S. Department of State.

If your name has changed on your previous passport then you must submit the legal document showing the name change. (Marriage certificate, divorce decree, etc.)

After filling out DS-82 "Renewal by Mail", follow the mailing instructions on the back of the form.
